No credit card? No problem! We have three alternate methods of payment for our clients:

Purchase a “Prepaid Green Dot Visa or MasterCard”
Make a cash deposit into Private MD Labs' Bank of America account
Pay by Money Order or Cashier’s Check

How to purchase a “Prepaid Green Dot Visa or MasterCard”

A “Green Dot Visa or MasterCard” works like a credit card, this card can be purchased at many retail stores, including CVS, Walgreen’s and Wal-Mart. The “Prepaid Green Dot Visa or MasterCard” unlike a Gift Card can be used over and over again. You simply load as much or as little on the card as you want and then use the card as you would a regular credit card.

To use your Prepaid Visa or MasterCard, simply complete your order online and enter the information related to your prepaid card in the credit card information.

Please Note: The Green Dot Cards do require you to register the card with the issuing financial institution. You will need to call or go online to register the Green Dot Card prior to using it. You will have to provide personal information when registering the card.

How to make a cash deposit into Private MD Labs

Once you have chosen the test(s) you would like to order, calculate the prices of the test. There are no hidden fees; the price listed on the site is the amount you will need to deposit in CASH into our account.

Go to any Bank of America with the exact amount of CASH for your order. Unless you are in the state of GA, ask for an out of state deposit slip.
Enter the following information on the deposit slip:

            Business Name: Private MD Labs
            Address: 1140 Hammond Drive, I-9100, Atlanta, GA 30328
            Bank of America Account Number: 0032 6307 7507

Enter the amount of your deposit in the area for cash on the deposit slip and the total deposit on the total line.
Give the cash and the deposit slip to the bank teller and wait on your receipt. Keep the receipt for your records.
Contact our office 877-283-7882 to notify our staff of your deposit. You will be asked for the following information:

Name to be listed on the lab requisition (doctor's order for test)
Date of Birth
Gender
Phone number to reach you in case of technical issues with email
The lab location that is most convenient for you
The test(s) that you would like to order
The email address where you would wish your requisition and results sent

Once the deposit has been confirmed, your order will be processed and you will receive your requisition (doctor’s order) and any special instructions related to your test(s) via secure email for you to print and take to the lab.

If you do not have access to a printer, contact our office again at 877-283-7882 so that we can fax the requisition to the chosen lab.

How to pay by Money Order or Cashier’s Check

Once you have chosen the test(s) you would like to order, calculate the prices of the test. There are no hidden fees; the price listed on the site is the amount you will need to get a cashier’s check or money order made out for.

Send the cashier’s check or money order along with the following information to our address listed below:

Name to be listed on the lab requisition (doctor's order for test)
Date of Birth
Gender
Phone number to reach you in case of technical issues with email
The lab location that is most convenient for you
The test(s) that you would like to order
The email address where you would wish your requisition and results sent

We suggest you send the money order in a traceable package, such as UPS, FedEx or delivery confirmation via US Postal Service.
