New model of care improves treatment for diabetics

Diabetics may be able to improve their HbA1c test scores by seeking treatment at a patient centered medical home, which is a model of primary care that is becoming popular for its ability to reduce costs while improving patient outcomes. A new study has found that diabetics in these systems are much more likely to improve their condition.

In a patient centered medical home, doctors take a much more proactive approach to treating patients, particularly those with chronic diseases. Rather than waiting for someone to get sick, the physician tries to counsel them on healthy lifestyle habits that keep them healthy.

The model of care proved to be effective in the current trial. Researchers enrolled 25 patients with diabetes into patient centered medical homes. These individuals were much more likely to achieve weight loss and blood sugar goals. Additionally, they were less likely to get sick or die during the course of the study.

"Diabetes is one of the most costly of chronic diseases, accounting for $174 billion in medical care each year in the United States, with the cost of care for patients with diabetes averaging 2.3 times higher than similar patients without diabetes," said Robert Gabbay, who led the investigation. He added that the model of care could help control costs.
