You should fast for 12 hours before collection of the specimen. Fasting is defined as no consumption of food or beverage other than water for at least 12 hours before testing. Some tests in this panel may exhibit interference when sample is collected from a person who is consuming a supplement with a high dose of biotin (also termed as vitamin B7 or B8, vitamin H or coenzyme R). Samples should not be collected during or soon after the administration of heparin. If using a testosterone cream please be sure you have not rubbed any into the antecubital area of your arm for the last 24 hours as it can give elevated results. For accurate test results, no ejaculation during the 24 hours preceding the test. " According to Beckman Coulter, the NAC interference should be insignificant by 12 hours after completion of the initial loading dose of an IV infusion treatment regimen consisting of an initial loading dose of 150 mg/kg administered over 1 hour, a second dose of 50 mg/kg administered over 4 hrs and a third dose of 100 mg/kg administered over 16 hrs. In patients receiving therapy with high biotin doses (ie, > 5 mg/day), no sample should be taken until at least eight hours after the last biotin administration. The assay manufacturer Beckman Coulter advises: "N-Acetyl Cysteine (NAC), when administered in therapeutic concentrations (for the treatment of acetaminophen overdose), has been determined to interfere with assays for cholesterol, uric acid" where "NAC interference may lead to falsely low results. Patient should be cautioned to stop biotin consumption at least 72 hours prior to the collection of a sample.
